Sycamores are invited to submit COVID-19 vaccine record

Indiana State University is asking all ISU students and employees to voluntarily submit proof of COVID vaccinations as part of the Sycamore Community Health Initiative. This information will be considered private and will only be used for contact tracing purposes or as part of overall, non-identifiable reporting about campus vaccination rates, and determining which students and employees are required to be tested weekly for COVID-19 as explained in the Sycamore Community Health Initiative. These vaccination records will be kept confidential consistent with federal and state privacy laws Voluntary disclosure will have no implications on an employee’s job status.

How to Submit Vaccine Records

Sycamores can upload their vaccination record by:

  • Signing into the new ISU portal
  • Clicking the ISU Apps folder on the left-hand column
  • Clicking the Covid-19 Vaccination Record App
  • Completing the survey*
  • Securely uploading a picture of official documentation of vaccination

Indiana Vaccination Portal

Official documentation is your vaccination card from the administrator of your vaccines or a vaccination certificate from the state immunization registry. Indiana residents can download their vaccination certificate and click through to the Indiana Vaccination Portal.

*The survey will ask for the manufacturer and dates vaccines were administered.

Each submission will be reviewed by Indiana State University Contact Tracing/Human Resources. When a vaccination record submission is complete, a request will be submitted to remove that employee from the daily email reminders to complete the survey and weekly COVID-19 testing.

If a fully vaccinated ISU student or employee is a close contact of a person diagnosed with COVID-19, and has been notified by ISU Contact Tracing or State or Local Health Departments, the daily Health Assessment Survey will be required for two weeks.
